A referral is not required — anyone may simply walk in during open hours, no appointment needed. This form is here for agencies that would like to make a formal referral so our team can watch for the person and plan a .

Every person has the right to choose their own provider. Club Serenity will never require someone to use a particular treatment program, recovery house, or service in order to receive help here, and choosing a different provider will never affect the support they get from us.

Our team confirms this again with the person when they arrive.
