A referral is not required — anyone may simply walk in during open hours, no appointment needed. This form is here for agencies that would like to make a formal referral so our team can watch for the person and plan a .

We only share information back with a referring agency once the person signs a — records are protected under .

We read referrals during Drop-In Center open hours and aim to acknowledge them within 24 hours. This form is not monitored around the clock — in a medical emergency, call 911.
